Preeclampsia is a high-risk pregnancy complication that often triggers anxiety, particularly in primigravida mothers facing a planned Cesarean section (SC) for the first time due to situational crisis and knowledge deficits. This study aimed to describe the implementation of preeclampsia education and birth preparation—delivered via a single 30-minute intervention—in reducing anxiety levels among third-trimester pregnant women scheduled for a Cesarean section. A descriptive case study method utilizing the nursing process approach was employed with Mrs. D (28 years old, G1P0A0, 38 weeks' gestation), who presented with preeclampsia, obesity, and an oblique fetal malposition at the OB-GYN clinic. The intervention consisted of anxiety reduction measures integrated with preeclampsia education, birth preparation, and deep-breathing relaxation exercises, conducted in a single 30-minute session. Success was evaluated using the Indonesian Nursing Outcome Standards (SLKI) indicators for anxiety levels. Evaluation results showed that, subjectively, the patient reported feeling calmer; objectively, she appeared more relaxed and was able to recall most of the health information provided. A reduction in anxiety levels was observed in the indicators for verbalized confusion (initial score 3/moderate, improved to 4/sufficiently decreased) and restless behavior (initial score 3/moderate, improved to 4/sufficiently decreased); however, verbalized worry remained at a score of 3 (moderate) due to the complexity of the high-risk pregnancy condition, resulting in the anxiety issue being only partially resolved. Health education regarding preeclampsia and birth preparation, integrated with deep-breathing relaxation techniques, was effective in fostering cognitive understanding and stabilizing the patient's anxiety response. Providing this pre-operative education can serve as an effective independent nursing intervention to facilitate the physical and psychological preparation of high-risk pregnant women prior to undergoing a Caesarean section
