Stroke can cause limb weakness, limited mobility, and dependence in daily activities, thereby increasing the risk of pressure injuries due to prolonged immobilization and bed rest. Families play an important role in assisting patients with repositioning; therefore, adequate knowledge and skills are needed to prevent pressure injuries. This community service activity aimed to improve family knowledge and skills in pressure injury prevention through education and repositioning practice for stroke patients. The activity was conducted at dr. H. Moch. Ansari Saleh Regional General Hospital, Banjarmasin, South Kalimantan, involving families of stroke patients with limited mobility. The activity used an approach involving education, demonstration, direct repositioning practice, and evaluation. Participants’ knowledge was assessed using pre-test and post-test questionnaires, while repositioning skills were assessed using an observation checklist. Analysis was conducted by comparing knowledge scores before and after education and assessing skill performance. The results showed that the mean knowledge score increased from 6.25 on the pre-test to 7.50 on the post-test, representing an increase of 1.25 points. The median score increased from 6.0 to 7.5, while the score range changed from 5–8 to 7–8. Skill observation showed that 21 of 24 observed opportunities were performed correctly, corresponding to an overall achievement of 87.5%. Patient preparation, communication, and patient safety reached 100%, while repositioning technique, use of pillows or assistive devices, and ensuring comfort after repositioning each reached 75%. The activity demonstrated positive changes in family knowledge and skills. Repeated demonstrations, practice, and follow-up evaluation are needed to support sustained pressure injury prevention in family-based stroke care.
